Our Aged Care Help at Home

Personal Care

To help you maintain your personal care routine, we can help you with bathing, showering, toileting, dressing/undressing, getting in and out of bed, washing and drying your hair, shaving, or with your medication schedule. If you are experiencing challenges with impairments or continence, we can provide you with continence advisory services.

Home Assistance

To help you live well at home, we can provide support in preparing meals, getting meals delivered  (excluding the cost of food), household chores, garden maintenance, and shopping. Ensuring the safety of your home is paramount. As your aged care provider, we can provide advice on areas of concern in the home, support with making modifications, and help you access technical assistance for major home modifications.

Transport and Socialisation

To help you stay connected and attend appointments, we can provide support with transportation and arrange social activities. Transportation assistance may include arranging a driver service, providing transport vouchers, or driving you to your appointment or social activity. Socialisation support may include organising your attendance at a group-based activity at a centre or arranging for a visitor to make a social call.

Aged Care Packages

The Australian Government offers four levels of Home Care Package funding to meet your required care needs.

Level 1

Level 1 Aged Care Package 

Entry-level care designed to provide you with a little extra help around the home or with transportation. This will involve a few visits per week by carers.

What services are included in Aged Care Help at Home services?

As an Aged Home Care Provider, Jewish Care help you access an Aged Care Package that will meet your unique care needs. Your package provides you with a range of support services, including personal care, domestic assistance, shopping, meal and food preparation, transportation, access to health professionals, Allied Health support, and access to devices, OT Aids, and equipment to make you safer and more comfortable at your own home.

Can the services be customised to suit my individual preferences and schedule?

Once you have an Aged Care Package, you will speak with our Aged Care Assessment Team so they can gather in-depth information about your needs, goals, preferences and schedule. This information will be passed on to an Aged Care Caseworker who will develop a person-centred, holistic plan for you.

What safety measures are in place to protect the well-being of clients receiving services at home?

To protect the well-being and safety of our clients, JewishCare implements a comprehensive client care process. When a client joins JewishCare, they will be visited by their Care Manager at home to develop a care plan tailored to their needs and goals. Throughout their journey with JewishCare, the client will be visited by their Care Manager every month to discuss their care and speak through any concerns. Support Workers working day to day with our clients will always provide feedback to the Care Manager if they have any concerns, and the Care Manager will visit the client’s home if there is ever an incident, such as a fall.

Can the services be adjusted or scaled up/down based on changing needs?

Yes, our aged care services can be adjusted based on changing needs. Care Managers seek regular feedback and can amend services based on individual needs and goals within the parameters of the client’s available funds and what is allowed within the program.

Are any financial assistance or government funding available to help cover the costs?

While the Australian Government subsidies a high portion of residential aged care costs, you may be required to contribute towards the cost of your care depending on your own income and assets. Fees and charges levied by residential aged care providers including JewishCare are government-regulated and vary depending on factors including whether you receive an aged pension or are a self-funded retiree. 

Access to your Aged Care Package is determined by Services Australia within Centrelink. They will determine whether you need to pay an income-tested fee and if so, how much this will be. For eligible holocaust survivors, home support under the Claims Conference is free.
