Here’s how to sign-up and receive your offer

Do you have an approved Home Care Package ready to sign up with us or, or are you happy to move from your current provider to JewishCare?

Call us on 0478 137 567, or fill out the form below.

Three

When your sign-up is complete, we will send you your vouchers. Vouchers will be provided across an 8-week period.

*This offer is only valid for our Home Care Package program until 30th June 2024. Valid for our Home Care Package program for someone with an approved ACAT package. Vouchers are provided as 4 x $100 vouchers over an 8-week period after the service agreement has been signed.

Allied Health Support services are available to you through your Aged Care Package. Our registered nurse and occupational therapist conduct in-home, comprehensive assessments, and provide professional recommendations when your care requires it.
